| The Forum Small Business of the Year Award recognizes a small business for their outstanding business achievements and civic contributions. A small business as defined by the Small Business Administration is a service business and retailer with annual revenues of $3.5 million or less, manufacturer with fewer than 500 employees, and wholesaler employing fewer than 100 workers. To nominate a business for the award, please complete the form below.
You may nominate more than one business, however, please complete one form for each nominee. Nominee(s) will be notified of their nomination and asked to submit more detailed information. Businesses nominated should be at least 3 years old.
